2. PhilHealth Membership Documents and ID
3. (Optional) Results of the most recent Laboratory Tests:
    a. CBC, SGPT/SGOT, Liver Function Tests
    b. Hepa A & B Tests
    c. PTB Tests
    d. Chest Xray
    e. Toxoplasmosis Test
    f. STD (syphilis, gonorrhea)
